Notice to Trainees – Maintenance of Training Log Book

All trainees who have commenced the four-month pharmacy training program are kindly informed that they are required to maintain a personal training log book throughout the training period.

Please note that this is a self-maintained log book. There is no prescribed format issued by the Council. Trainees are expected to make brief daily entries (approximately 5–6 lines per day) summarizing their learning and activities during the training.

These entries should reflect the practical exposure and learning experiences gained during the training. Trainees may include the following where applicable:

  • Something new learnt during the day
  • A reflection on the work performed
  • Feedback received from the supervising pharmacist
  • A patient encounter or case observation
  • A new extemporaneous compound or preparation learnt/ made

Trainees may also document experiences related to the following professional activities:

  • Interpretation of prescriptions and detection of errors
  • Patient counselling on medication use
  • Drug information provision
  • Inventory management and storage of medicines
  • Participation in patient counselling
  • Drug stock management activities
  • Exposure to compounding or special preparations
  • Observations of pharmacist–patient interactions
  • Identification and resolution of prescription errors
  • Adverse drug reaction reporting
  • Drug interaction checks
  • Patient counselling cases (e.g., diabetes, hypertension, asthma medications)
  • Public health services (e.g., health promotion or screening programs)

The purpose of the log book is to encourage reflection on daily learning and to document the range of professional activities observed or performed during the training period.

Trainees are advised to maintain their log books regularly and accurately throughout the training program.
