Collecting Internal Pharmacist’s Certificates
- Rs. 5,000 /= paid payment slip
- CMCC Account no. 167-1-001-2-0020892 peoples bank Town Hall branch (You can pay at any people’s Bank branch islandwide)
- National Identity card or Driving License
- Original Internship (Experience) Certificate
- Mondays only from 9am to 3pm.
Collecting Internal Pharmacist’s Academic Transcripts
Transcript can be apply though the email (cmcc.fmc@gmail.com) or handover the following documents to the CMCC office
Document to submit
- Request Letter (Mentioning the training school : Colombo/NIHS)
- Rs. 10,000 /= paid payment slip
- CMCC Account no. 167-1-001-2-0020892 peoples bank Town Hall branch (You can pay at any people’s Bank branch islandwide)
- Copy of the proficiency certificate
- Copy of NIC
