- Certified True Copy of the Diploma in Midwifery Certificate issued from Department of Health Services
- English Translation of Birth Certificate or Driving License or Passport Copy
- Rs. 5000/= Paid Payment slip
- CMCC Account no 167-1-001-2-0020892 peoples bank Town Hall branch(You can pay at any people’s Bank branch islandwide)
- Only on Wednesday from 9 am to 3pm
