Please bring the Original Indenture Form  Rs. 2000 /= paid payment slip CMCC Account no. 167-1-001-2-0020892 people’s bank Town Hall branch (You can pay at any People’s Bank branch islandwide)  Tuesdays only from 9am to 2pm.

Collecting External Pharmacist’s Academic Transcript

• Transcript can be applied though the email (cmcc.fmc@gmail.com) or handover the following documents to the CMCC office
• Request Letter
• Rs. 10,000 /= paid payment slip
• CMCC Account no. 167-1-001-2-0020892 people’s bank Town Hall branch (You can pay at any People’s Bank branch islandwide)
• Copy of the efficiency certificate
• Copy of National Identity Card

Collecting External Pharmacist’s Certificates

• Rs. 5000 /= paid payment slip
• CMCC Account no. 167-1-001-2-0020892 people’s bank Town Hall branch (You can pay at any People’s Bank branch islandwide)
• National Identity card or Driving License 
• Mondays only from 9am to 2pm.
